ABSTRACT

SUMMARY: For treating cruciate ligament injuries, especially for characterizing the mechanics of the tunnel in cruciate ligament reconstruction, correctly understanding the bony information of the attachment area of the cruciate ligaments is significant. We studied 31 knee joints of middle-aged Chinese adults using the P45 sheet plastination technique, focusing on the attachment areas of the cruciate ligaments, especially the bony structures. The trabeculae at the attachment area were distributed radially and extended deep into the medial wall of the lateral condyle of the femur. However, in the anterior part of the intercondylar eminence, the trabeculae of the anterior group were parallelly arranged along the tendinous fibers of the anterior cruciate ligament, while the trabeculae of the posterior group were parallelly arranged along the perpendicular direction of the anterior cruciate ligament fibers. Similarly, at the attachment area of the lateral wall of the medial condyle of the posterior cruciate ligament, the trabeculae extended radially toward the deep medial condyle. Deep in the posterior part of the intercondylar eminence, the trabeculae were arranged longitudinally. In the anterior part of the intercondylar eminence, the trabeculae were parallelly arranged along the perpendicular directions of ligament fibers. The distribution patterns of the trabecular at the attachment areas of the cruciate ligaments at the ends of the femur and tibia were different. This difference should be considered when orthopedic surgeons reconstruct anterior cruciate ligaments.

ABSTRACT

BACKGROUND: Porous silicon carbide has excellent physical and chemical properties and can be used to repair complex shapes and long weight-bearing bone defects, but it has no biological activity and cannot promote bone remodeling and integration. OBJECTIVE: A tantalum (Ta) coating was deposited on the surface of the porous silicon carbide scaffold to evaluate its biological activity. METHODS: The chemical vapor deposition method was used to deposit a Ta coating on the surface of the porous silicon carbide scaffold. The optimal experimental parameters were explored by setting different gas reaction flow rates and temperatures. The Ta coating was prepared with the best experimental parameters to test the mechanical properties of porous Ta. Human osteoblasts were co-cultured with the Ta coating specimens, and were observed by scanning electron microscopy after 3 and 7 days of cultivation. RESULTS AND CONCLUSION: (1) After the optimization of experimental parameters, the best experimental conditions were controlled as follows: deposition temperature of 1 050 °C, hydrogen flow rate of 180 mL/min and chlorine flow rate of 100 mL/min. (2) The compressive strength of the Ta coated specimen was (61.4±3.2) MPa; the yield strength was (45.8±2.9) MPa; and the elastic modulus was 4.8 GPa. (3) Scanning electron microscopy demonstrated that after 3 days of co-cultivation, a large number of cells adhered to the surface of the porous silicon carbide scaffold that is coated with Ta and the porous structure. Some of the cells protruded from the pseudopod and were connected to each other. After 7 days of co-cultivation, the cell protrusions fused to form a sheet covering the surface of the porous Ta. In the porous structure, the cells protruded from the pseudopods, cross-linked to each other across the pores, secreted in the matrix, and coated the microparticle structure, gradually spreading in the pores. (4) The results show that the Ta coating on the surface of porous silicon carbide scaffold has good mechanical properties and biological activity.

ABSTRACT

BACKGROUND: The implantation of titanium alloy prosthesis would produce stress shielding to damage mechanical microenvironment of bone tissue. This might affect normal bone remodeling process. Interference fit was used to generate preload to reconstruct mechanical microenvironment in this area. It may have important theoretical significance and clinical application value to promote bone remodeling in the mid-long-term after surgery. OBJECTIVE: To analyze the promoting effect of preload on osseointegration of porous titanium alloy prosthesis. METHODS: Fourteen male New Zealand rabbits were divided into two groups. The three-dimensional (3D) printed titanium alloy prosthesis was implanted into the outside of right femoral condyle with interference fit in experimental group (n=7). Rabbits in the control group (n=7) were feed free. Femurs at the surgery side were extracted at 12 weeks after surgery. Micro-CT scanning and 3D reconstruction were performed to measure bone density and trabecular morphological parameters around the prosthesis. Animal experiment was approved by Animal Ethics Committee of Tianjin Institute of Medical Science (approval No. IMPS-EAEP-Z-2019108-01). RESULTS AND CONCLUSION: (1) Bone mineral density was reduced significantly in the experimental group compared with the control group (P 0.05). (3) The results showed that preload could promote osseointegration of porous titanium alloy prosthesis. However, the structural characteristics of the in-growth bone were worse than healthy bone. This might lead to a high fracture risk in the mid-long-term after surgery.

ABSTRACT

OBJECTIVE: To explore the effect of cadmium on bone formation and its mechanism in male mice. METHODS: i) The specific pathogen-free C57 BL/6 J wild-type male mice were divided into control group and cadmium exposure group, with 10 mice in each group. Mice in the cadmium exposure group were intraperitoneally injected with 2 mg/kg body weight cadmium chloride twice a week for eight weeks, and mice in the control group were intraperitoneally injected with the same amount of 0.9% sodium chloride solution. After that, the bone mineral density and bone microstructure of the femur of mice were detected by a high-resolution microcomputed tomography scanner. ii) Human SV40-transfected osteoblast cells(hFOB1.19) were divided into control group and cadmium exposure group. The cadmium exposure group was treated with 0.5 μmol/L cadmium chloride solution, cells in the control group were given an equal volume of α-low-limit minimal medium. After culture, the differentiation and mineralization ability of hFOB1.19 cells were analyzed by alkaline phosphatase(ALP) and alizarin red staining, respectively. Cell viability was examined by CCK-8 assay. The protein expression of Janus kinase 2(JAK2), total signal of transducers and activator of transcription 3(tSTAT3) and phosphorylated signal transducers and activator of transcription 3(pSTAT3) was detected by Western blotting. The expression of osteoblastic marker genes ALP, Runt-related transcription factor 2(RUNX2) and osteocalcin(OCN) was detected by real-time quantitative polymerase chain reaction.RESULTS: Compared with the control group, the average bone mineral density decreased(P<0.01), bone volume fraction decreased(P<0.05), the bone trabecula thickness became thinner(P<0.01), the number of bone trabecula decreased(P<0.05), trabecular bone spacing increased(P<0.05) in the femur of mice in cadmium exposure group. The viability of hFOB1.19 cells was decreased [(100.0±10.8)% vs(49.1±8.2)%, P<0.01]. The differentiation ability of osteoblasts was reduced and the mineralization was inhibited. The relative expression levels of JAK2 and pSTAT3 in cells decreased(all P<0.05) and the relative expression levels of osteoblast marker genes ALP, RUNX2 and OCN decreased(all P<0.01).CONCLUSION: Cadmium can induce mice bone loss, which may be due to its inhibition of osteoblastic function by reducing the expression of JAK2 and STAT3 proteins.

ABSTRACT

Objective To study fluid flow within alveolar bone under orthodontic and occlusal loading, so as to provide references for understanding the regulatory mechanism of bone remodeling during orthodontics. Methods An animal model for orthodontic tooth movement on rats was first constructed. The finite element model of tooth-periodontal ligament-alveolar bone was established based on micro-CT images and the strain field in alveolar bone under orthodontic or constant occlusal loading was analyzed. Then finite element model of alveolar bone was constructed from the bone near the cervical margin or apical root of mesial root. The fluid flow in this model under orthodontic and cyclic occlusal loading was further predicted by using fluid-solid coupling numerical simulation. Results The fluid velocity within alveolar bone cavity mainly distributed at 0-10 μm/s, and the fluid shear stress (FSS) was mainly distributed at 0-10 Pa. FSS on the surface of alveolar bone near the apical root was higher than that close to the cervical margin. Conclusions FSS at different levels could be produced at different location within alveolar bone cavity under orthodontic and cyclic occlusal loading, which might further activate biological response of bone cells on the surface of trabeculae and finally regulate the remodeling of alveolar bone and orthodontic movement of tooth. The results provide theoretical guidance for the clinical treatment of orthodontics.

ABSTRACT

Objective: To explore the effect of icariin on early steroid-induced osteonecrosis of the femoral head in rabbits. Methods: Fifty mature New Zealand rabbits (weighing, 2.5-3.0 kg) were randomly divided into control group ( n=10), model group ( n=20), and experimental group ( n=20). The rabbits of model and experimental groups were injected with lipopolysaccharide and methylprednisolone to establish the animal model of early steroid-induced osteonecrosis of the femoral head. The rabbits of experimental group were feeded with icariin solution once a day for 6 weeks since the first injection of methylprednisolone, whereas the rabbits of control and model groups were given normal saline at the same time points. The left femoral heads were removed after 6 weeks and gross morphological features were evaluated. Micro-CT scan was performed to analyze the trabecular microstructure with the following parameters: trabecular bone volume to total volume (BV/TV), trabecular number (Tb.N), trabecular thickness (Tb.Tn), and trabecular separation (Tb.Sp). The Micro-CT scan was also converted to three-dimensional reconstruction images for observation. HE staining was applied to observe the trabecular structure and morphological changes of osteocytes and marrow adipocytes. It was also used to determine whether the samples of femoral heads occurred osteonecrosis based on the criteria for pathological diagnosis, and calculate the rate of empty lacunae. Results: Seven rabbits died during the study, and 9, 16, and 18 rabbits in the control, model, and experimental groups, respectively, enrolled the final analysis. Compared with control group, the femoral head collapse and trabecular breaks were more obvious, and the trabeculae were sparse with irregular arrangement in the model group according to the results of gross observation, Micro-CT scan, and three-dimensional reconstruction images. But in the experimental group, the surface of femoral head was slight shrinking without obvious collapse, and the degeneration of trabecular structure was mild. According to bone microstructures analysis, the Tb.N, Tb.Tn, and BV/TV of femoral head in model and experimental groups were lower than those in control group, while the Tb.Sp in the model and experimental groups were significantly higher. The Tb.N, Tb.Tn, and BV/TV of femoral head in experimental group were higher than those in model group, while the Tb.Sp in the experimental group was significantly lower. The differences between groups were all significant ( P<0.05). In the model group, HE staining showed that the number of osteocytes reduced, the number of empty lacunae increased, and the marrow adipocytes piled up in the space between femoral trabeculae, some even mashed together like a cyst. In the experimental group, the trabecular structure was still relatively complete compared with model group, no obvious apoptosis of osteocytes was observed, the size and number of adipocytes were basically normal. None of the animals in control group occurred osteonecrosis of the femoral head based on the criteria for pathological diagnosis, and the incidence of osteonecrosis were 81.3% (13/16) in the model group and 66.7% (12/18) in the experimental group, and the difference was not significant ( P=0.448). The rate of empty lacunae of osteonecrotic femoral heads in the model group was 33.1%±1.4%, which was higher than that in experimental group (18.9%±0.8%) and in control group (12.7%±1.5%), and the differences between groups were significant ( P<0.05). Conclusion: The icariin has a protective effect on the early steroid-induced osteonecrosis of the femoral head in rabbits, which can decrease osteocytes apoptosis, improve the bone microstructure, and delay such disease processes.

ABSTRACT

BACKGROUND: At present, related finite element models have been used to simulate the femoral fracture, and the effects of loading rate, loading angle and cancellous bone on the fracture of hip have been discussed. However, the fracture simulation of trabecula is still lack of relevant research. OBJECTIVE: To simulate the biomechanical process of osteoporotic trabecular compression fracture in ovariectomized rats. METHODS: The right femur of ovariectomized rats was scanned at the distal end of femur by Micro-CT. The microstructure parameters and three-dimensional model of the region of interest of the rat femur were obtained. After geometric optimization in Geomagic Studio, they were pretreated in HyperMesh 14.0, including volume mesh division, setting material property parameters, boundary conditions, setting load of 1 200 N, acting time of 2 ms, and they were calculated in LS-DYNA software. RESULTS AND CONCLUSION: (1) The bone trabeculae in the region of interest showed uneven spatial distribution. (2) The bone trabeculae with small volume and small number first presented deformation fracture, and the plate shape and bone trabeculae with large volume finally demonstrated fracture collapse. (3) The change trend of von Mises stress was roughly the same as that of bone trabeculae fracture collapse. The fracture collapse process of bone trabeculae in the region of interest included vertical collapse and horizontal torsion, in which the degree and rate of horizontal torsion were lower than that of vertical collapse, making the size and rate of cross-section torsion angle less than that of coronal plane angle. (5) The increase and peak value of shear stress of failure unit were smaller than Von-mises stress. (6) These results indicate that fracture collapse of bone trabecula is a complex process, including deformation and angle of different planes.

ABSTRACT

Fluid shear stress (FSS) caused by interstitial fluid flow within trabecular bone cavities under mechanical loading is the key factor of stimulating biological response of bone cells. Therefore, to investigate the FSS distribution within cancellous bone is important for understanding the transduction process of mechanical forces within alveolar bone and the regulatory mechanism at cell level during tooth development and orthodontics. In the present study, the orthodontic tooth movement experiment on rats was first performed. Finite element model of tooth-periodontal ligament-alveolar bone based on micro computed tomography (micro-CT) images was established and the strain field in alveolar bone was analyzed. An ideal model was constructed mimicking the porous structure of actual rat alveolar bone. Fluid flow in bone was predicted by using fluid-solid coupling numerical simulation. Dynamic occlusal loading with orthodontic tension loading or compression loading was applied on the ideal model. The results showed that FSS on the surface of the trabeculae along occlusal direction was higher than that along perpendicular to occlusal direction, and orthodontic force has little effect on FSS within alveolar bone. This study suggests that the orientation of occlusal loading can be changed clinically by adjusting the shape of occlusal surface, then FSS with different level could be produced on trabecular surface, which further activates the biological response of bone cells and finally regulates the remodeling of alveolar bone.

ABSTRACT

Objective To study the relationship of the tibial plateau subchondral trabecular bone (STB) microstructure and the cartilage degeneration with the lower limb alignment based on individual trabecula segmentation (ITS) and histology analysis in knee osteoarthritis (OA). Methods Hip-knee-ankle (HKA) angles were measured on the full-length lower extremity films of patients before total knee arthroplasty (TKA). The tibial plateau excised from the TKA were collected for micro-CT scanning and ITS analysis. The cartilage degeneration was evaluated by histology. The relationship between the HKA angle and the changes in microstructural parameters of STB and cartilage degeneration were analyzed. ResultsThe plate, rod and axial bone trabecular volume fraction (BV/TV, pBV/TV, pBV/TV), ratio of trabecular plate versus rod (P/R), plate trabecular number density (pTb.N), plate trabecular thickness (pTb.Th), trabecular plate surface area (pTb.S), trabecular rod length (rTb.L), and plate-plate and plate-rod junction density (P-P Junc.D, P-R Junc.D) of the subchondral bone of the tibial plateau were significantly related to the cartilage degeneration OARSI score and the HKA angle. The greater the deviation of the lower limb alignment, the greater the number of subchondral trabeculae, the thicker the trabeculae, the greater the bone mass, the stronger the connectivity, especially the plate trabeculae on the affected side of tibial plateau, and the higher the OARSI score of cartilage degeneration. Conclusions Abnormal lower limb alignment may cause abnormal microstructure of the plate and rod STB of the tibial plateau by changing the stress distribution of the knee, especially the significant increase and thickening of the plate trabecular and axial trabecular bone, which may be an important risk factor that further aggravates the degeneration of articular cartilage and the progress of OA. Therefore, lower limb alignment correction with surgical intervention and improving STB with bone metabolism agents may efficiently contribute to preventing cartilage damage and mitigate OA progression.

ABSTRACT

Background and objectives: Moderator band is a specialized bridge present between the base of the anteriorpapillary muscle and interventricular septum. It carries the right branch of the bundle of HIS with it. The band isknown to prevent the over distension of the right ventricle during the diastolic phase. There is a need of lot ofresearch and studies on the septomarginal trabecula as it proves to be important clinically. Here we measuredthe length, breadth, height, angle with the interventricular septum, and the superficial marking of Moderatorband on the sternocostal surface of the right ventricle is done. This paper describes the morphological variationsfound in its origin and insertion.Materials and Methods: 15 formalin-soaked cadaveric hearts were taken from the Department of Anatomy ofKVG Medical College, Sullia. The foetal hearts, the hearts with gross pathology, if any are excluded from the study.Result: The morphometric parameters are tabulated and the morphological variations are noted. The results arecompared with the results of the previous studies on the moderator band. Conclusion: It is confirmed that the thickness of moderator band varies at its origin and insertion. The surfacemarking of moderator band on sternocostal surface of the heart will help in locating the moderator band throughthe echocardiography and during cardiac surgeries on right ventricle. The possibility of second moderator band, multiple origin or insertion is more than 20%.

ABSTRACT

Aim To study the influence of Sodium fer-ulate ( SF) on bone metabolism in glucocorticoid–in-duced osteoporosis rats. Methods Thirty cases of fe-male Wistar Rats(3-month-old) were divided into con-trol group, model group and SF group ( low-dose group, middle-dose group, high-dose group ) by ran-domized block design. Double fluorochrome labeling with calcein was performed before necropsy. The left tibia was taken for bone histomorphometry. Results In static parameters, the proximal tibia cancellous bone trabecular thickness, trabecular quantity and area ratio were significantly reduced in model group compared with control group;while compared with model group, those were increased in middle and high-dose SF group. Trabecular separation degree was increased in model group compared with control group, while it was decreased in middle and high-dose SF group compared with model group. In dynamic parameters, the calcula-tion parameters of cancellous bone mark perimeter rate and the bone formation rate were increased in model group compared with control group, in middle and high-dose SF group the bone formation rate was in-creased compared with model group. In bone cells, os-teoclast number per mm, osteoblast number per mm, percent osteoblast surface perimeter and percent osteo-clast surface perimeter were increased in model group compared with control group. In growth-plate, the thickness of growth-plate was increased in model group compared with control group. In bone cells and growth-plate there was no statistical significance between treat-ment group and model group. Conclusion This study demonstrates that SF can increase bone mass and im-prove bone structure, which may be related to the im-provement of bone formation. SF is effective for GIOP in rats.

ABSTRACT

Objective To quantify the stress and strain of trabecular bone with microdamage/microfracture by using micro-CT and micro-finite element (μFE) analysis, so as to investigate the effects of osteoporosis on relationship between trabecular stress and microdamage/microfracture. Methods Two cylindrical specimens of healthy and osteoporotic acetabular trabecular bones were scanned by micro-CT to build 3D μFE analysis models. The uniaxial compression on two specimens was simulated under frictionless displacement boundary condition. The trabecular stress, trabecular strain, microdamage and microfracture under different apparent strains were calculated through nonlinear μFE analysis. Results For both the healthy and osteoporotic trabeculae under apparent strain of 0.05%-0.50%, the undamaged trabecular stress was under 50 MPa, while the damaged trabecular stress was above 110 MPa. Compared with the osteoporotic trabeculae, the mean stress of the healthy trabeculae was relatively higher, but the maximum stress of the osteoporotic trabeculae was higher. Trabecular damage occurred in both the healthy and osteoporotic trabeculae, while the healthy trabeculae showed more microdamage, and microfracture occurred in the osteoporotic trabeculae. Conclusions Within the scope of apparent strain, the healthy trabeculae can withstand a higher stress with more trabecular microdamage, and microfractures might occur in the osteoporotic trabeculae under the high stress.

A cerebral abscess is defined as a collection of pus within the cerebral parenchyma. Though rare in infants, it is common among young patients and the elderly. The clinical manifestations are varied. The predominant etiological agents include Streptococci, staphylococci and klebsiella. A patient is diagnosed based on the clinical picture, the physical exam and complementary tests. Treatment options depend on the stage of the disease, its size, the causative agent, and the neurological condition of the patient. A case is presented of a four month old infant with a history of an upper respiratory tract infection three months prior to his admission. One month later, he exhibited a high fever of 38-39 °C with irritability and a tense anterior fontanel. The CT scan revealed a hypo dense lesion in the left frontal lobe with significant mass effect and midline shift. The lesion was punctured on two consecutive days and 80 ml of yellowish pus was removed on both occasions. Four days later, a left fronto-temporal craniotomy and an ipsilateral frontal lobectomy was performed, with complete excision of the capsule. Antibiotic therapy was continued for three weeks. The post op MRI confirmed the complete excision of the lesion and disappearance of the surrounding edema. Recovery was satisfactory

0.05).[Conclusion]The values of three-dimensional parameters in old femoral neck fracture patient are different with that of normal persons in the loading region of the femoral head although there is no difference in BMD values between them. This may provide new evidence to predict osteoporotic femoral neck fracture.

ABSTRACT

The partial splenectomy was performed on the basis of the arterial distribution in order to reduce complications. The spleen consists of the various histologic components that have different functions. But we can not find the reports whether the distribution of red pulp, white pulp and trabecula is uniform or not according to the regions of the spleen. We used 15 spleens from Korean[male 8, female 7 and age 17-74]. The volume was measured with the mass cylinder. The 1cm3 blocks were selected in 5 different regions. Whole splenic slices by celloidin embedding were made from two spleens. The point counting method with eyepeice reticule was used for the measurement of the distribution of red pulp, white pulp and trabecula. The results were as follows : 1. The volume of the spleen was ranged 45-158ml and the individual difference was marked. The male spleen was larger than female`s [p<0.05]. 2. The ratio of red pulp, white pulp and trabecula of the paraffin embedding preparations was 83.2, 10.9, and 5.9% respectively. There was no significant difference in the distribution of the histologic components among five regions[upper, hilum, middle, outer, and lower]. 3. The distribution of red and white pulps in whole splenic slices was not uniformity. The ratio of red pulp, white pulp and trabecula in whole splenic slices was 80.1, 13.4 and 6.5% respectively. These results showed no significant difference with those of five regions of the spleen.

ABSTRACT

PURPOSE--To evaluate possible morphological changes in chronic chagasic of the right bundle trabecular branch (RB) with right branch block (RBB) and to draw clinicopathological correlations. METHODS--Eight RBB chronic chagasic septo-marginal trabeculae (SMT) (group A), six left branch block (LBB) chagasic SMT (group B) and six SMT from non-chagasics with no heart disease (group C) were analyzed. Every SMT was completely embedded in paraffin, sub-serially sectioned to the end of the paraffin-block and the sections were processed for pathological study. RESULTS--Right bundle branch trabecular segment mononuclear infiltrate and/or fibrosis were found for 87.5 (7/8) of group A, 50 por cento (3/6) of group B and 16.6 por cento (1/6) of group C. Moderate mononuclear infiltrate and fibrosis were noted respectively for 3 and 1 cases, from group A. For the remaining group A cases and for all the group B and C cases the mononuclear infiltrate and fibrosis were slight. CONCLUSION--The frequency and degree of RB lesions might explain some of RBB. On the other hand, the absence of lesions in one case RBB and the slight degree of RB lesions in chagasics with LBB and in one non-chagasic case indicate that sometimes it is not possible to establish electrocardiographic-pathological correlations after TSM histological examination

ABSTRACT

Objective To investigate the effects on structure of madibles and femurs in ovariectomized rats. Methods Ovariectomy was performed on female SD rats, which were killed 8 weeks later. The mandibles and femurs were taken out as samples to be analyzed by bone histomorphometry and to calculate trabecular area(Tb.Ar), trabecular width(Tb.Wi), trabecular separation(Tb.Sp). Results After ovariectomy the osteopotic trabecular changes had occured in the mandibles and femurs of the rats. There was a significant difference between Tb.Ar, Tb.Wi and Tb.Sp of the mandibles and femurs in ovariectomized rats and sham-ovariectomized(P
